Addressing the Social Determinants of Health in Primary Care

The EveryONE project is an initiative from the American Academy of Family Physicians focused on helping primary care physicians learn about and address the social determinants of health. Their toolkit includes a guide on the social determinants of health as well as screening tools that primary care physicians can use to screen their patients for social and environmental concerns that may be affecting their health.
