CDC’s Transforming Health Page

The Transforming Health: Patient-Centered HIV Prevention and Care website is a new resource from the Centers for Disease Control and Prevention. The focus of this site is to aid in the reduction of new HIV infections and improve the health of transgender individuals living with HIV. The site contains information for clinicians, care teams and other healthcare providers as well as resources for people who are transgender. The provider-focused resources include those that can help with the delivery patient-centered HIV care, increase HIV testing and prevention, and improving retention in HIV care.
